Able Australia is conducting this survey to determine what percentage of our workforce has been vaccinated against COVID-19. This information will help us better support our clients and you as our staff. This information will also help us to plan for potential COVID-19 outbreaks.

Disclosure of your vaccination status is voluntary. Your vaccination information will be confidential and only disclosed as required to support the work you do. In the case of an outbreak, it may be shared with your line manager and the Quality & Innovation Team. It will be stored by the People & Culture Team.

The aggregated information (total number of staff or percentage of staff vaccinated) may be communicated to the Board and outside organisations such as the NDIA and the NDIS Quality and Safeguards Commission.

Your vaccination status will not be shared with clients or their families/gaurdians, or with your colleagues.
